Case Story:
MD Yousuf Azad is a 46-year-old male diagnosed with ALK-positive metastatic lung adenocarcinoma, a type of non-small cell lung cancer (NSCLC). Adenocarcinoma originates from the gland-forming cells of the lung and is one of the most common and aggressive forms of lung cancer. In his case, the disease was diagnosed in August 2021 after he presented with persistent dry cough. Imaging (HRCT thorax) revealed a large mass measuring 8.2 × 6.2 cm in the left upper lobe of the lung, extending to the pleural surface and hilum. Biopsy confirmed infiltrating adenocarcinoma with signet ring cells. Further evaluation showed spread to mediastinal lymph nodes and the adrenal gland, indicating Stage IV metastatic disease. Molecular testing showed ALK positivity, a genetic alteration that drives cancer growth but also allows treatment with targeted therapy.
Since October 2021, he has been on targeted therapy with Ceritinib 450 mg once daily, which is an ALK inhibitor used to control disease progression. The treatment has resulted in partial response initially and stable disease on subsequent scans. He is currently clinically stable with good performance status and no major complications; however, metastatic lung cancer requires lifelong continuous treatment, regular scans, laboratory monitoring, and cardiac evaluation to prevent progression and manage potential side effects. If treatment is interrupted, there is a high risk of rapid disease progression.
Mr. Azad is a poor fruit vendor and the only earning member of his family. Due to his illness, the family’s financial condition has become extremely fragile. The yearly cost of his life-saving targeted therapy, along with investigations, consultations, and supportive care, is approximately ?7,00,000 per year, which is far beyond their financial capacity. Continuous financial support is essential to ensure uninterrupted treatment and to sustain his survival and quality of life.
